COMMUNITY HOUSING
 

Residential Supports provides assistance with acquisition, retention, or improvement in skills related to activities of daily living, such as personal grooming and cleanliness, bed making and household chores, eating and preparation of food, and the social and adaptive skills necessary to enable the individual to reside in a non- institutional setting.

Residential Support Levels:

  • Level 1 and Level 1 AFL - H2016;

  • Level 2 and Level 2 AFL - T2014;

  • Level 3 and Level 3 AFL - T2020;

  • Level 4 and Level 4 AFL - H2016HI;

  • Level 5 and Level 5 AFL – T2016HI

COMMUNITY LIVING & SUPPORT
 

Community Living and Support is an individualized service that enables the waiver beneficiary to live successfully in his/her own home, the home of his/her family or natural supports and be an active member of his/her community. Community Living and Support enables the person to learn new skills, practice and/or improve existing skills. Areas of skill acquisition are: interpersonal, independent living, community living, self-care, and self-determination.

Community Living and Support provides supervision and assistance for the person to complete an activity to his/her level of independence. Areas of support consist of assistance in monitoring a health condition, nutrition or physical condition, incidental supervision, daily living skills, community participation, and interpersonal skills.

LONG-TERM COMMUNITY SUPPORT
 

Long-Term Community Supports (LTCS) is a community-based comprehensive Medicaid service for adolescents and adults (ages 16 and older) with intellectual/developmental disabilities (I/DD). It provides individualized services and supports to enable a person who would otherwise be institutionalized in an Intermediate Care Facility.

COMMUNITY NETWORKING
 

Community Networking (individual or group) is a service that provides individualized day activities that support the individual’s definition of a meaningful day in an integrated community setting. A goal of this service is to be inclusive of individuals with disabilities within the community, therefore it is provided separate and apart from the home of the individual and that of the provider.

DAY SUPPORTS/DAY ACTIVITY
 

Day Supports/Day Activity is a day treatment facility that provides supervision and an organized program during a substantial part of the day in a group setting to individuals who are mentally ill, developmentally disabled or have substance abuse disorders.

The service is designed to support the individual's personal independence and promote social, physical and emotional well-being through activities such as social skills development, leisure activities, training in daily living skills, improvement of health status, and utilization of community resources.

SUPPORTED EMPLOYMENT
 

Supported Employment--Individual is a service which involves arranging for and supporting paid work for an individual, age 16 or older, in a job which would otherwise be done by a non-disabled worker.

 

The service includes intensive involvement of staff working with the individual in the work setting or in support of the individual so that the individual's employment can be maintained in a normal community environment.

NONEMERGENCY MEDICAL TRANSPORTATION
 

(NEMT) is transportation by wheelchair van, or litter van for those who cannot use public or private transportation. NEMT is an important benefit for Medicaid beneficiaries who need to get to and from medical services but have no means of transportation. NEMT is only utilized to access non-emergency services.

NEMT is a transportation service provided to individuals who are not in an emergency, but need more medical assistance and specialized equipment than other transportation options can provide.

RESPITE SERVICES
 

Respite services provide periodic or scheduled support and relief to the primary caregiver(s) from the responsibility and stress of caring for the individual. NC Innovations respite may also be used to provide temporary relief to individuals who reside in Licensed and Unlicensed AFLs, but it may not be billed on the same day as Residential Supports.

 

This service enables the primary caregiver to meet or participate in planned or emergency events, and to have planned time for him/her and/or family members.
